School Registration
Aloha and Welcome to Honoka'a High and Intermediate School
Below is a list of items you need to enroll your child if you are new to Hawaii
Copy of Birth Certificate
Photo ID (of the parent or guardian)
A document with your physical address verifying you live in our district (for example, mortgage statement, rental agreement, utility bill, TV bill)
TB clearance (completed in Hawaii within the last year)
Completed Enrollment Forms (click here for the forms). Please note if page 6 section 2 is checked, do not complete page 7
Most recent report card and exit grades from previous school (these grades will be used to average with the grades from the remaining weeks of school to receive credit)
Immunization records from previous school (In most cases they provide you a form when you exit)
Transcript from previous school
Release form from previous school
For families transferring from another public school within Hawaii you do NOT need to submit 4 and 7 from above
After the enrollment forms are submitted, we will schedule an intake meeting to determine course selections. This meeting could be held at the same time the forms are turned in. Please call Mr. Hanks to schedule a meeting or for more information at 808-313-7021. Please note if you are a student with an IEP you will need to schedule an intake meeting with our Special Education Program Manager, Michelle Martin at 808-313- 7091.
